The Rapid Emergency Assessment and Care Team (REACT) are looking for an enthusiastic, highly motivated and experienced Physiotherapist to join their leadership team! REACT's interdisciplinary way of working within Emergency Medicine's Assessment Areas and Short Stay wards aims to avoid hospital admissions, whilst providing high standards of care and discharge planning. The team are passionate about safely discharging patients back into the community and we achieve this through comprehensive holistic therapy assessments and working alongside external community partners.
In addition to working clinically across our footprint, the successful candidate will play a key role in driving forward change whilst offering leadership and operational support to our REACT service and wider Emergency Medicine and Allied Health Professional Groups. This will support many opportunities to develop skills in leadership, service improvement and change management, as well as opportunities to gain knowledge in patient flow, operational processes to support this and integrated care systems. Applicants should be dynamic, innovative, have experience of working within Admission Avoidance &/or Community pathways and be ready to work within a fast-paced environment, for which a high level of problem solving and clinical reasoning will be required.
Responsibilities- To manage a caseload of patients with a varied range of conditions +/- complex needs using evidence-based practice with client centred principles in order to assess, plan, implement and evaluate interventions.
- To effectively identify and utilise appropriate discharge pathways and services in order to promote a 'home first' approach and support patient flow.
- To provide specialist advice, teaching and training to other members of the MDT across the Emergency Medicine footprint regarding the management of patients presenting to Urgent and Emergency Care services.
- To support the operational delivery of the REACT service and workforce management.
- To support the learning and development of junior staff and students.
- To participate in the planning, development and evaluation of the REACT service, including holding responsibility for defined projects.
- To participate within a 7 day working model, including extended hours.
