We are seeking a compassionate and motivated Band 5 Community Nurse to in Swindon . This role involves delivering high-quality nursing care to patients in their own homes or community settings, supporting them to maintain independence and achieve their healthcare goals. You will work collaboratively within a multidisciplinary team, providing a range of nursing interventions and supporting the holistic needs of patients with acute, chronic, or complex health conditions. The role requires strong clinical skills, excellent communication, and a passion for delivering patient-centred care in a community setting. Key Responsibilities Clinical Care Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care. Conduct holistic assessments of patients’ physical, emotional, and social needs to develop and implement individualised care plans. Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
