We are seeking a compassionate and motivated Band 5 Community Nurse to in West London . This role involves delivering high-quality nursing care to patients in their own homes or community settings, supporting them to maintain independence and achieve their healthcare goals. You will work collaboratively within a multidisciplinary team, providing a range of nursing interventions and supporting the holistic needs of patients with acute, chronic, or complex health conditions. The role requires strong clinical skills, excellent communication, and a passion for delivering patient-centred care in a community setting. Key Responsibilities Clinical Care Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care. Conduct holistic assessments of patients’ physical, emotional, and social needs to develop and implement individualised care plans. Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes. Provide health education and advice to patients and carers to support self-management and promote independence. Community-Based Care Work autonomously in patients homes and community settings, ensuring safe and effective care delivery. Manage a daily caseload, prioritising tasks and responding to changes in patients conditions. Use digital tools and systems to plan, record, and report on care delivery in line with organisational policies. Team Collaboration Work closely with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care. Participate in multidisciplinary team meetings and case conferences to support care planning and review. Act as a patient advocate, ensuring their preferences and needs are central to care planning and decision-making. Professional Development Commit to continuing professional development (CPD) to maintain and enhance clinical skills and knowledge. Participate in clinical supervision, reflective practice, and training sessions provided by the organisation. Support the development of students, healthcare assistants, and less experienced colleagues through mentoring and guidance. Compliance and Governance Maintain accurate and timely clinical records in accordance with professional and organisational standards. Adhere to local and national policies, guidelines, and regulations, including infection control and safeguarding protocols. Promote and maintain a culture of safety, quality, and continuous improvement. Person Specification Essential Criteria: Registered Nurse (Adult) with current NMC registration. Clinical experience in a relevant setting, such as community, primary care, or acute services. Ability to work independently and manage a caseload in a community environment. Strong clinical assessment and decision-making skills. Excellent communication, interpersonal, and organisational skills. A full UK driving licence and access to a vehicle for work purposes (if applicable). A commitment to patient-centred care and promoting independence. Desirable Criteria: Previous experience in community nursing. Knowledge of long-term condition management, palliative care, or wound care.
