What You’ll Be Doing
- You’ll deliver person‑centred functional and environmental assessments and interventions that empower clients to maintain independence and improve quality of life.
- Working in clients’ homes, you’ll provide specialised equipment solutions and pressure care strategies.
- Managing a wide variety of client presentations across multiple community aged care funding programs including Home Care Packages, Commonwealth Home Support Program, Transitional Care Program and Palliative Care.
- Conducting in‑home assessments and developing tailored treatment plans to help clients meet their goals. This includes simple and complex equipment prescription, major and minor home modifications, and client education.
- Collaborating within a supportive statewide Occupational Therapy team of 90+ OTs with decades of collective experience, as well as working locally with a diverse multidisciplinary team of nurses, care workers and support staff.
