To lead the strategic development, implementation, and continuous review of person-centred documentation across all services, ensuring it reflects best practice, regulatory standards, and the lived experience of people who draw on our care and support. This role will champion the use of Nourish digital care planning software to embed personalised, outcome-focused documentation that supports safe, effective, and compassionate care.

Lead the design, review and implementation of digital person-centred care documentation, ensuring it is strengths-based, inclusive, and aligned with current legislation and guidance (e.g., CQC, CIW, Skills for Care, NICE).
Ensure digital documentation supports the principles of personalisation, dignity, choice, and control.
Ensure digital documentation supports the different models of service across the organisation.
Collaborate with operational teams, people who use services, and families to co-produce documentation formats and content.
Digital Integration (Nourish)
Act as the organisational lead for Nourish (digital electronic care planning) implementation and optimisation. Work with Nourish and internal stakeholders to configure templates, workflows, and reporting tools that reflect person-centred practice. Provide guidance and training to staff on effective use of Nourish for care planning, daily documentation, and outcomes tracking.
Quality Assurance and Improvement
Monitor the quality and consistency of documentation across services through audits, feedback, and data analysis. Identify areas for improvement and lead change initiatives to enhance documentation quality and usability. Ensure electronic documentation supports regulatory compliance and is inspection-ready.
Training and Support
Work collaboratively with Learning and Development colleagues to develop and deliver training materials and webinars on person-centred documentation and digital care planning. Provide ongoing support and coaching to teams to embed best practice. Act as a subject matter expert for documentation queries and system use.
Qualifications and Experience
- Minimum of 5 yearsβ experience in adult social care or health care settings.
- Strong understanding of person-centred care principles and regulatory frameworks.
- Experience of implementing digital care planning systems, ideally Nourish within a care setting.
- Excellent communication, facilitation, and project management skills.
- Strong understanding of the principles of co-production.
- Strong understanding of current Health and Social Care Legislation and evidence of implementing quality improvement.
- Excellent presentation and IT Skills.
- Ability to work independently.
- Familiarity with data protection and digital record-keeping standards.
- Experience in co-production or service user engagement.
- Knowledge of quality improvement methodologies.
- Professional degree or post graduate certificate in a relevant field.
