Hackney Council is currently recruiting for an experienced Adults Team Manager to join the Integrated Hospital Discharge Service. This is a key leadership role within a fast‑paced, multi‑disciplinary environment, focused on ensuring safe, timely, and effective hospital discharges for Hackney residents.

About the Role

As Team Manager, you will provide strong operational leadership across the service, managing a team responsible for complex discharge planning. You will play a critical role in reducing Delayed Transfers of Care (DTOC) while ensuring residents and their families receive high‑quality, person‑centred support throughout their discharge journey. You will work collaboratively with health and social care partners, driving effective multidisciplinary working to achieve positive outcomes.

Key Responsibilities

  • Lead, manage, and support a team of social workers and practitioners
  • Oversee complex discharge planning and decision‑making
  • Drive performance to reduce delayed transfers of care (DTOC)
  • Ensure timely completion of statutory assessments and interventions
  • Promote high‑quality, outcome‑focused practice across the service
  • Work closely with multidisciplinary teams to coordinate effective discharge pathways
  • Support residents and families throughout the hospital discharge process

About You

We are looking for a confident and experienced leader who:

  • Has extensive experience in hospital discharge and complex case management

  • Has a sound understanding of statutory frameworks, including: Mental Capacity Act, Care Act, Adult Safeguarding
  • Possesses excellent communication skills and the ability to engage effectively with multidisciplinary teams

  • Is committed to delivering high‑quality, person‑centred outcomes
