Our client Calderdale Hiring is looking for a Social work Team Leader to join their Hospital discharge team.

Principal duties and responsibilities:

  • Ensure that effective systems are in place to screen referrals against eligibility criteria.
  • Work with partners to ensure that referrals are dealt with creatively, deploying staff and other resources appropriately.
  • Prioritise and allocate work to appropriate staff based on the complexity and urgency of a service request.
  • Utilize the resources of the whole team and other skills within the overall service.
  • Ensure the effective management and deployment of resources within budgets to meet agreed policy objectives and service priorities.
  • Communicate effectively with the team about budget management.
  • Ensure that the review and evaluation of Support Plans are carried out in accordance with practice guidelines, making appropriate adjustments to reflect changing needs.
  • Ensure a systematic approach to the assessment planning, implementation and review of health needs in accordance with policies and procedures.
  • Advise and consult with the appropriate Team Manager & Service Manager on expenditure for community care services that exceed current guidelines.
  • Ensure that staff have fully informed and involved users and carers in the assessment process and in agreeing personal budgets, packages of care, and ensuring copies of assessment and care plans are provided.
  • Promote professional development of staff through regular supervision, identify training opportunities, work in accordance with agreed outcomes in Performance Appraisal Scheme interviews, and assist qualified nurses and social workers to maintain professional registration.
