Hampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Queen Alexander Hospital, Portsmouth. The role will be primarily duty which will include triaging, resolving issues in a fast-paced environment, safeguarding, managing expectations, speaking with patients and their next of kin, and managing a task list.

Responsibilities:

  • Working with the Advanced Assessment Team (AAT).
  • Completion of care act assessments and mental capacity act assessments as needed.
  • Building professional MDT networks with key areas in the hospital.
  • Risk assessment work and close liaison with community teams around high intensity users.
  • Close links with our short-term service and community bed providers to stream people rapidly to the services to avoid admissions and truncate stays.
  • Use of HCC systems to provide AAT with collateral information to enable rapid decision making.
  • Represent AAT on wards, as required including participation on stranded patient sessions if required.
