Location - Wales (Swansea, Cardiff, Shropshire, Newport, Llandudno, Llandrindod Wells and surrounding areas)

Job Purpose: To provide high-quality, person-centred care and support to service users, assisting with daily living activities, promoting independence, dignity, and wellbeing in line with care plans and organisational standards.

Key Responsibilities:

  • Assist service users with personal care (washing, dressing, toileting, grooming).
  • Support mobility and transfers safely using appropriate equipment.
  • Assist with meal preparation and feeding where required.
  • Monitor and report changes in physical or mental health conditions.
  • Maintain accurate and timely care records.
  • Support medication administration (where trained and permitted).
  • Promote independence, choice, and dignity at all times.
  • Follow safeguarding, infection control, and health & safety procedures.
  • Work collaboratively with nurses, families, and multidisciplinary teams.
