We are seeking a dedicated and compassionate Support Worker/Healthcare Assistant to join our team. The successful candidate will provide high-quality care and support to individuals with Mental Health issues, Learning Disabilities, and Autism. This role offers a rewarding opportunity to make a meaningful difference in the lives of vulnerable individuals while working in a supportive and collaborative environment.
Key Responsibilities- Provide person-centered care and support to service users in line with their individual care plans
- Assist service users with activities of daily living while promoting independence
- Support service users to access community activities and appointments
- Monitor and accurately record service users wellbeing, including medication administration when required
- Implement positive behavior support strategies
- Maintain accurate and up-to-date documentation
- Work collaboratively with the wider multidisciplinary team
- Respond appropriately to emergencies and challenging situations
- Actively participate in team meetings, supervision, and training opportunities
- Experience working with individuals with Mental Health issues, Learning Disabilities, and/or Autism
- Valid Enhanced DBS check (or willingness to undergo one)
- Strong knowledge of Safeguarding practices and procedures
- Valid UK driving license (driving is a requirement for this role)
- Excellent communication and interpersonal skills
- Ability to work flexibly, including evenings, weekends, and bank holidays as required
