Location: Havant
Skills and Experience Required:
- Provide support and therapy to individuals and families experiencing mental health issues such as anxiety, depression, schizophrenia, or substance misuse, using a strengths-based approach.
- Conduct assessments (psychosocial, risk, care needs) to establish diagnoses and identify required support.
- Develop and monitor individualised care plans, in collaboration with clients and multidisciplinary teams.
- Undertake crisis intervention and risk management, safeguarding vulnerable individuals using frameworks such as the Mental Health Act, Mental Capacity Act, or Care Act.
- Advocate for clients' rights and coordinate with community, statutory, and voluntary services to ensure access to resources and promote social inclusion.
- Provide therapy and counselling through one-to-one or group sessions using evidence-based models such as CBT or DBT.
- Maintain accurate records, case notes, progress documentation, and adhere to confidentiality and ethical standards.
- Support carers and families, offering psychoeducation, emotional support, and inclusion in recovery planning.
