Hampshire CC are seeking a case worker in their CART (Community/Care and Adults Resource Team).
Location: Fareham
Rate: 23 (umbrella)
Contract Length: 6 months
Hybrid Working: 1 day a week in the office, 4 days at home (once training complete)
The CART Team is the contact centre for all Adult, Health and Care enquiries, dealing with nearly 100,000 enquiries a year by telephone, online forms and web chat. We operate 5 days a week. Our aim is to resolve a minimum of 85% of enquiries that come into CART.
As a Case Worker you will be part of a supportive and friendly team working with other social/health care professionals managing incoming enquiries by telephone, online forms and webchat. You will assess residents' needs and provide them with support and guidance to enable them to maximise their wellbeing. Through your person-centred approach, you'll focus on the elements of care, support and treatment that matter most to the person, their families and carers. Using strengths-based practice, you'll focus on what a person can do, whilst finding innovative, alternative community resources to meet their needs.
After your initial office-based training period of up to 4 weeks you will be able to take advantage of our hybrid working model, giving the opportunity to work from home approximately four days per week (service demand/training needs permitting).
In this role you will:
- Support individuals by completing assessments and reviews in a way that promotes a person's well-being and maximises their independence whilst mitigating risk.
- Provide information and advice, drawing on different sources, in accessible formats, that meet individual needs.
- Recognise when a case and/or situation develops an increased level of risk and know how to respond and when to seek advice and support.
- Know how to identify and respond to potential safeguarding situations and understand how to report them.
- Manage your workload effectively and prioritise appropriately.
- Build and maintain positive working relationships with multi-disciplinary colleagues.
- Take responsibility to prepare and participate in supervision i.e. regular 1:1 meetings with a senior member of the team to discuss how your role has contributed to the team's priorities.
- Support colleagues in achieving the operational priorities for the team.
