Location - Wales (Swansea, Cardiff, Shropshire, Newport, Llandudno, Llandrindod Wells and surrounding areas)

Job Purpose:

To support individuals with learning disabilities, autism, mental health conditions, physical disabilities, and additional support needs to live safely, independently, and with dignity. The role focuses on promoting choice, inclusion, personal development, and active participation in the community.

Key Responsibilities:

  • Provide tailored emotional, social, and practical support in line with individual care plans.
  • Assist individuals with learning disabilities, autism, and mental health needs in their daily routines.
  • Support the development of independent living skills including cooking, cleaning, shopping, and managing money.
  • Encourage and enable participation in community activities, education, employment, and social events.
  • Promote independence, confidence, and personal empowerment at all times.
  • Provide personal care support when required, following care plans.
  • Support safe administration of medication where trained and authorised.
  • Observe and report changes in wellbeing, behaviour, or mental health.
  • Raise safeguarding concerns in line with policies and procedures.
  • Keep accurate records, daily notes, and progress updates.
