About the role
PCRS Lewisham based in Forest Hill is looking for a full time experienced GPSC (GP Shared Care) Substance Misuse Recovery worker. Immediate start required for this role.
The successful applicants will manage a caseload of adult service users in a GPSC setting, i.e., working at GP surgeries as well as at our main office in Forest Hill, Lewisham, delivering evidence-based interventions in line with individual recovery plans. This may include liaising closely with a full range of workers and skills within the team as well as family members and significant others as appropriate.
Experience of working in a GPSC (GP Shared Care) setting and with clients on OST (Opiate Substitute Treatment) is essential. Knowledge of case management recording system, System One would be ideal.
As a GPSC Recovery Coordinator, you will deliver Substance Misuse specific knowledge, interventions and support, harm minimisation, and help set personalised recovery plan goals to support the service user achieve their goals in a safe and supportive way. You will also collaborate closely with key partners like GPs and other social care professionals to achieve the best health and wellbeing outcomes for individuals.
Work with service users towards detoxes both community and in patient as well as rehabilitation centres. Working collaboratively with key partners and colleagues to achieve the best health and well-being outcomes for individuals.
