Social Worker required for work within BANES Discharge to Assess Team. 37 hours a week with rates up to £35-38 per hour for experienced candidates. Contract minimum 3 months. Flexible hybrid working.

We are looking for compassionate, proactive, and motivated individuals to join the D2A Team across Bath & North East Somerset (BANES). The D2A pathway supports individuals to leave hospital safely and continue their assessment and recovery in the most appropriate setting. The team works closely with health, social care, therapy, and community partners to support safe hospital discharge and ongoing recovery within community settings. Team members play a key role in helping people regain independence and achieve the best possible outcomes following discharge from hospital.

Key Responsibilities
  • Coordinate safe and timely hospital discharges
  • Complete strengths-based assessments within community settings
  • Support individuals to maximise independence and recovery
  • Work collaboratively with NHS, social care, therapy, and voluntary sector colleagues
  • Develop person-centred care and support plans
  • Monitor outcomes and elevate concerns where appropriate
  • Maintain accurate and timely case records

You must be SWE registered. Ideally a driver with access to own car and able to reside and work in the UK.

Qualifications
  • Social Worker with relevant experience.
  • Registered with Social Work England (SWE).
  • Ability to drive and own vehicle, or access to transport.
  • Eligible to work in the UK.
