Care Coordinator to join our growing Extra Care team at Queensway Court.

About the Role

As our Care Coordinator, you'll be at the heart of our service, ensuring every client receives safe, reliable and person-centred care.

You'll play a key role in ensuring continuity of care, supporting service growth and helping our team deliver the outstanding experience our clients deserve.

  • What You'll Be Doing
  • Coordinating client care schedules using our Nourish care management system.
  • Ensuring continuity of care by matching Caregivers with clients.
  • Managing daily rotas, annual leave, sickness cover and schedule changes.
  • Supporting new client starts, Caregiver introductions and shadow shifts.
  • Building positive relationships with clients, families and Caregivers.
  • Working closely with Orbit Housing, local NHS teams, Social Workers and other healthcare professionals.
  • Maintaining accurate records and ensuring compliance with CQC standards.
  • Supporting recruitment, onboarding and training coordination.
  • Contributing to business growth, quality improvement and service development.

What We're Looking For

You'll be someone who

  • Has experience in care coordination, scheduling or care management.
  • Is highly organised with excellent attention to detail.
  • Can remain calm and make good decisions under pressure.
  • Enjoys communicating with people from all backgrounds.
  • Is confident using IT systems and Microsoft Office.
  • Has a genuine passion for delivering exceptional customer service.
  • Shares our values of compassion, respect and professionalism.

Experience within domiciliary care, Extra Care or another health and social care setting would be advantageous.
