Key Responsibilities

  • Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
  • Activity planning: develop flexible, engaging activity plans that promote autonomy and personal growth.
  • Health and lifestyle: support individuals in maintaining healthy lifestyles and managing their health needs.
  • Communication: facilitate effective communication and maintain detailed, person-centered records.
  • Medication management: oversee medication processes and support individuals in self-administration.
  • Safety and environment: maintain a safe, clean living environment and adhere to health and safety protocols.
