In this role, you will serve as a key link between clients, their families, and our care staff, ensuring the delivery of high-quality care tailored to each individual's needs. Your responsibilities will encompass managing care plans, coordinating schedules, and overseeing the day-to-day operations of care delivery. As the Care Coordinator, you will play a crucial role in optimising service delivery, enhancing client satisfaction, and fostering a positive work environment for our support staff. Responsibilities:
- Lead the coordination of care for clients.
- Schedule and manage staff assignments to ensure consistent and efficient care delivery.
- Act as the primary point of contact for clients and their families, addressing inquiries and concerns promptly.
- Facilitate communication between clients, families, and caregivers to promote a cohesive care approach.
- Assist with staff training and development to ensure high care standards are met.
