Job overview

Work under the direction of a Registered Nurse (RN) or Advanced Clinical Practitioner (ACP) in SDEC, providing personal care and assistance to patients within defined parameters, carrying out a limited range of clinical duties and supervising the work of others.

Responsibilities

  • Work effectively as a team member within multidisciplinary teams in SDEC/frailty team.
  • Assist the RN/ACP in delivering care to a designated patient group, prioritising care as required.
  • Determine patient comfort by talking and listening, building rapport.
  • Assist patients with personal hygiene, toilet assistance, assess health safety and wellbeing, escalating appropriate care.
  • Assist with feeding and drinking for patients unable to do so.
  • Maintain patient dignity with tact, empathy, and persuasion.
  • Report any changes in patient condition to the RN/ACP.
  • Act as a role model for all colleagues.

Person specification

Experience

  • Previous experience in a clinical/ward/care setting.
  • Ability to work independently caring for a group of patients.
  • Experience completing clinical documentation.
  • Completed Care Certificate or equivalent (e.g., NVQ).

Knowledge

  • Awareness of Trust Values and Priorities.
