The Acute Frailty Team is seeking a highly motivated individual to develop our services. The ideal candidate has an interest in improving patient care within frailty and actively contributes to education and training to upskill the local workforce.

Main Duties:

  • Delivery of person‐centred care, screening for frailty and conducting comprehensive geriatric assessment (CGA).
  • Facilitate early identification and focused interventions for all adults with frailty presenting to our Emergency Department.
  • Support the development of a frailty pathway that follows patients through their clinical journey.
  • Close collaboration with community services and partners, expanding frailty services beyond traditional models.

Working at the Trust:

The Trust operates three main hospitals: Furness General Hospital, Royal Lancaster Infirmary, and Westmorland General Hospital, plus community sites in Millom, Queen Victoria Hospital, and Ulverston. The job is based at Furness General Hospital in Barrow.
