UHMB in Barrow-in-Furness is seeking a Band 5 Care Coordinator to join the Barrow Integrated Care Community (ICC) Clinical Team. You will coordinate care for patients with frailty and complex needs, assess needs, develop personalised care plans and signpost to health and social care services.
This role requires multidisciplinary collaboration, strong communication and IT skills, and willingness to travel across the ICC footprint.
