Job overview

The primary role of this post is to lead and provide a comprehensive, high quality clinical pharmacy services to Women’s Health and Maternity Services at the Trust. This includes delivering high quality, cost-effective, safe, efficient and timely pharmacy services for patients and wards within your speciality.

You will be responsible for developing and advising on pharmacy services that affect the phases of a UHCW patient’s journey. This will mean working with teams across the Trust and outside to ensure our patients medications are managed well. This will include screening all TTOs at ward level, supporting the Women’s and Maternity wards and leading/organising a pharmacy team to meet the demands of the service.

You will be required to develop and monitor key performance indicators that are patient outcome orientated.

A significant part of the role will be working with Clinical Service Leads, the pharmacy governance team, other clinical pharmacists and the Operational team to ensure that a suitably trained and equipped team of pharmacists and technicians are well led to provide ward based, patient centred models of care in Women’s Health.

The post holder will work within our Women’s and Children’s Pharmacy Team and will be required to provide cross cover for the Senior Pharmacist. The post holder will be required to build the required competencies to provide pharmacy services to Neonatal and Paediatrics in addition to Women’s Health in order to achieve this.

Main duties of the job

  • Provide effective clinical and medicines management service to patients in your areas at the Trust.
  • Lead, develop and deliver education and training in the pharmaceutical aspects of Women's Health and Maternity to other healthcare professionals.
  • To manage more junior rotational staff allocated to your areas including aspects of day-to-day line management and acting as a diploma tutor/designated supervisor as needed.
  • Identify areas within Women's Health and Maternity services where improvements in both quality and cost effectiveness of pharmaceutical care and prescribing may be encouraged.

Detailed job description and main responsibilities

  • Check prescriptions for clarity, safety and efficacy on wards, in dispensaries and when necessary take accurate drug histories to facilitate this. Query with and provide the prescriber with appropriate information to effect a change in the prescription when clinically required, to ensure patients’ safety and effective therapy.
  • Provide information to nursing and medical staff on allocated wards on any aspect of medicines policy ensuring safe practice. Provide information to patients on allocated wards or in dispensaries concerning their medication and answer any resulting questions or queries.
  • Provide when necessary, through checking, dispensing or supervision of technicians, an efficient medication supply service from any of the pharmacy departments, including the aseptic laboratory. To act as a floor team leader, directing and organising the teams and ensuring responsibilities are completed, as needed.
  • Ensure patients discharge medication is accurate and dispensed in advance of the patients’ discharge, by writing the patients’ medication on the GP discharge prescription and conveying to the GP all necessary information concerning the patient medication to ensure a safe transfer to primary care. Reducing pharmaceutical waste by utilising patients own drugs in the discharge process.

Qualifications

  • Masters Degree in Pharmacy + 1 year pre-registration training (or equivalent) Registered as a practising pharmacist with the GPHC Post-graduate Diploma in Clinical Pharmacy or equivalent experience Evidence of CPD Registered independent prescriber (or currently working towards)
  • Member of a professional organisation (e.g. RPS) Pharmacy or Pharmacy Practice related MSc

Experience

  • Suitable clinical pharmacy experience post registration (community, primary or secondary care) Suitable experience dealing with complex clinical situations Experienced at working with and influencing senior medical and nursing staff Training non-pharmacy healthcare professionals
  • Experience at a directorate pharmacist level (e.g. reviewing and developing clinical guidelines or medicines policies). Suitable senior hospital pharmacist experience Experience in line managing staff and conducting appraisals

Knowledge

  • Good sound Clinical Pharmacy Knowledge Up to date knowledge of Medicines legislation Able to Identify the costs associated with prescribing changes in your given clinical area Able to manage change To be able to present clinical and complex information to groups and individuals. To provide written information in a concise readable manner for patients and colleagues
  • Specialist knowledge of areas of clinical responsibility
