The post holder will be the point of contact for all administrative issues relating to patients’ pathway of care. They will work as part of a multi‑disciplinary team, providing expertise in proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly. The holder will supervise and allocate work to the Assistant Patient Pathway team and ensure high levels of patient and clinician satisfaction through an accessible, customer‑focused, knowledgeable point of contact. Standard Operating Procedures (SOPs) will guide the role, ensuring functions are carried out correctly within given timescales.
Responsibilities
- Process and log all referrals, including paper and electronic, in line with Trust Access & Administration Process Policy and specialty SOPs.
- Schedule new and follow‑up appointments, ensuring capacity is used efficiently and patients are seen by the most appropriate clinician on the first visit.
- Confirm all diagnostics and investigations are requested and results available for consultations, including those carried out at other Trusts.
- Coordinate appointments and procedures at external hospitals and organisations when required.
- Ensure outcomes of clinic attendance are accurately recorded on EPR by the Patient Pathway Assistant (cash‑up).
- Complete clinic cancellation forms and process them according to SOPs.
- Reschedule outpatient clinics as requested to meet waiting time targets.
- Use the Trust’s medical transcription system and ensure letters are processed in accordance with SOPs.
- Liaise with patient records staff, clinical colleagues, other admin teams and external organisations to ensure all medical notes, referral letters, results and discharge summaries are available for outpatient appointments and consultations.
- Identify patients on cancer pathways, liaise with Cancer Services MDT coordinators to ensure appropriate hand‑over.
- Use the 18‑Week Referral to Treatment (RTT) system to manage all outpatient and elective patient journeys.
- Validate the RTT Patient Tracking List (PTL) and investigate any incomplete pathways to ensure timely treatment and robust information.
- Identify and escalates any issues that compromise delivery of the 18‑Week RTT pathway, such as lack of capacity in outpatients or theatres.
- Ensure inter‑provider transfers are timely and proper paperwork is completed.
- Share patient pathway and diagnostic information with internal and external colleagues.
- Manage and monitor outpatient and theatre scheduling to maximize capacity.
- Keep Trust systems updated with patient pathway status information and maintain data quality.
- Proactively manage pathways to avoid breaches and resolve any issues.
- Provide communication and customer service support.
- Perform general administrative tasks, health records security & management, and engage in education and training/self‑development.
Person Specification
Essential
- NVQ Level 3 in Business Administration/Customer Care or equivalent experience; GCSE or equivalent.
- Experience using a full range of IT systems, accurate data entry, typing and checking skills.
- Ability to use own judgement to resolve problems and queries competently, including those sensitive and confidential in nature.
