This includes allocating care to Registered Nurses, Healthcare Support Workers and an Apprentice Senior Healthcare Support Worker, based on individual patient needs. This varied role combines office‑based coordination with clinical practice, including patient assessment and review, and effective use of digital systems to support safe and responsive care delivery. The post holder will also contribute to the development and support of junior staff, including mentoring responsibilities, and participate in the THH Traceability rota. Working closely with multidisciplinary teams across West Northumberland, the post holder will support integrated working, helping to improve access to high‑quality palliative and end‑of‑life care, ensuring patients and families receive compassionate, coordinated support when they need it most.
