As our services continue to grow in response to community need, we are looking to appoint a qualified and experienced Registered Nurse to join our Nursing Care Services as a Registered Nurse Care Coordinator. This is a rewarding opportunity to play a key role in coordinating compassionate, person-centred care and supporting patients and families at the most important times.

Main duties of the job

  • Working as part of the Nursing Service at Tynedale Hospice at Home, the Registered Nurse Care Coordinator plays a key role within the care coordination team, supporting the delivery of high-quality, person-centred care.
  • The role involves triaging referrals, coordinating care and managing a caseload, ensuring patients receive timely and appropriate support.
  • This includes allocating care to Registered Nurses, Healthcare Support Workers and an Apprentice Senior Healthcare Support Worker, based on individual patient needs.
  • This is a varied role combining office-based coordination with clinical practice, including patient assessment and review, and effective use of digital systems to support safe and responsive care delivery.
  • The post holder will also contribute to the development and support of junior staff, including mentoring responsibilities, and participate in the THH Traceability rota.
  • Working closely with multidisciplinary teams across West Northumberland, the post holder will support integrated working, helping to improve access to high-quality palliative and end-of-life care, ensuring patients and families receive compassionate, coordinated support when they need it most.
