For over 30 years, we have proudly supported people across Tynedale, Ponteland and West Northumberland, providing high-quality palliative and end-of-life care in the comfort of patients' own homes. Our services include home-based end of life nursing care, patient transport, complementary therapies and bereavement support. All of our services are provided free of charge.

General Statement about the role

Tynedale Hospice at Home (THH) Registered Nurses (RN) are integral members of the multi-disciplinary team providing supportive and palliative care to a range of patients with life limiting illnesses. Working within the THH Nursing Service, this role forms part of the RN Care Coordinator team, coordinating care through triage, ongoing patient management, and allocation of care to RNs, Hospice Support Workers (HSW) and Level 3 Apprentice Senior Healthcare Support Worker. The post incorporates both office-based and clinical responsibilities, including care coordination, patient assessment and review, utilisation of IT systems to support care delivery, mentorship of junior staff (including an Apprentice Senior Healthcare Support Worker), peer supervision/support when required and participation in the THH Traceability rota.
