We have an exciting opportunity for a paediatric Nurse to join our highly skilled and compassionate care team at Ty Hafan's hospice.
Main duties of the job
As a Registered Paediatric Nurse at Ty Hafan, you will deliver safe, high-quality clinical nursing care to children and young people receiving care in the hospice. Working as part of a supportive multidisciplinary team, you will help ensure continuity of care and the best possible outcomes for children and their families. You will use your professional skills to assess, plan and deliver individualised nursing care, providing evidence-based clinical interventions in line with current guidelines, regulations and organisational policies. A key part of the role is supporting families as well as children, delivering truly holistic, family-centred care. Job responsibilities
Job Purpose
The Registered Nurse works as part of a nurse led multi-disciplinary team and provides high quality, specialised and compassionate care to babies, children and young people living with life-limiting conditions through individualised child, and family centred care in our warm hospice environment. Registered Nurses work autonomously and unsupervised to undertake a wide range of clinical and therapeutic interventions following competency assessment and are responsible for ensuring the safe and effective care of their allocated child, in addition to supporting the wider care team objectives.

This role includes direct work with service users; colleagues across care services including with community and hospital-based teams; and with wider facilities team.
Main Duties and Responsibilities
- Provide safe and evidence based clinical care to children and young persons resident in the hospice, and to support the continuity of this care in the wider community when necessary.
- Work independently and autonomously within established Ty Hafan policies and procedures to provide care as delegated by the Lead Nurse.
- Facilitate the safe admission and discharge of children and their families attending for residential stays.
- Liaise with external professionals and the wider Ty Hafan team to support emergency clinical referrals.
- Advocate for participation of the child and their family in all aspects of care delivery, in collaboration with the multi-disciplinary team ensure that the total needs of child and family are met.
- Maintain a high level of clinical practice ensuring that all care delivered is in line with current regulations and guidance, provided by Ty Hafan and through professional bodies.
- Responsible for the assessment, planning and delivery of individualised nursing care.
- Responsible and accountable for the direct supervision of student nurses, and in-direct supervision of junior staff or Assistant Practitioners. Ensuring both appropriate delegation and provision of support in line with NMC guidance.
- Recognise and adhere to Ty Hafan's medicines management policy, autonomously administering medications to children in a homely manner as permitted.
- Autonomously provide a wide range of clinical interventions within the scope of your practice and clinical competency.
- Recognise and actively respond to the signs of clinical deterioration in children and young people. Utilising your skills and experience to prevent avoidable hospital admissions.
- Participate in standard setting, audit, data collection and quality review.
- Utilise equipment and resources in a cost-effective way, in line with the concept of prudent healthcare.
- Contribute positively to the creation and maintenance of a child and family-focused environment.
- Maintain and engage in Mandatory training, ensuring compliance remains within the agreed levels, working to agreed training deadlines, and adhering to organisation requirements enabling confident and competent practice.
- Actively participate in governance and health and safety processes, including completing risk assessments, highlighting concerns, and reporting all incidents, accidents and near misses promptly.
- Communicate effectively and in a timely and most appropriate manner with colleagues, children and families.
- Maintain accurate, clear and contemporaneous records and documentation in line with NMC requirements and Ty Hafan's record keeping policy.
- Take into consideration own level of confidence and competence, ensuring delegated tasks are within own level of experience and training.
Person Specification
Qualifications
- Registered General Nurse/BSc Nursing Child/LD trained to degree level.
- Current professional registration with the Nursing and Midwifery Council (NMC).
- Certified Practice Assessor.
Experience
- Experience of working with children with complex and specialists needs.
- Experience of working with external professionals and the wider MDT to ensure a seamless and holistic approach to care.
- Experience of assessing, planning, implementing and evaluating care requirements and care delivery to ensure care is consistently of high quality, safe, and is evidence based.
- Experience of working with children and/or adult safeguarding.
- Experience of working in paediatric palliative.
- Previous experience of non-invasive ventilation, Syringe drivers and transcription.
- Ability to assess, plan deliver and evaluate care for children with multiple complex needs.
