Covering the South, East and West Wales Ty Hafan offers a beautiful, newly refurbished working environment set in idyllic grounds a short distance from the metropolitan area of Cardiff. We have an exciting opportunity for a Family Support Worker to join our well-established Family Support Team within our Family Wellbeing & Outreach services. This is a full time, permanent role and is based at the hospice and in the community. Applicants will be required to work flexibly, working weekend and evenings as and when service need requires. About the role We are looking for someone who has experience in delivering and facilitating events. The role will organise and facilitate a range of events, both in the hospice and community settings based around service user participation, that meet the needs of either mums/ female carers; dads/ male carers or whole family groups, ensuring they are equitable across our catchment area and considering the wide range of familys needs. The role will also provide 1-1 low level emotional support to families referred to the hospice and through bereavement. Palliative care is defined as the total care of individuals from diagnosis of a life-shortening condition, beyond transition into adult services, or in providing extensive bereavement support. It is important to remember that paediatric palliative care is so much more than just end of life care, Ty Hafan holds enhancing quality of life at its very core.
