Overview
This exciting new development within Lincolnshire, based in Grantham, welcomes enthusiastic and experienced applicants to apply for a rare opportunity to be involved in shaping a brand new pilot service that will support individuals stepping down from acute mental health wards to transition into the community. The service will be delivered in partnership with Lincolnshire Partnership NHS Trust and will form part of the NHS Mental Health Pathway. The service will go live in October 2025, with a full training and induction programme for the team before go live.
With an emphasis on recovery working alongside specialist housing and financial-related support, you will need to work flexibly to manage a small caseload and help with the day-to-day running of the service.
Role Responsibility
Raising the bar for person-centred care, you will provide a range of recovery-focused support through key working sessions and group work. You will help people gain new skills and promote long-term recovery, enabling a positive impact on their lives.
You will have off-rotor responsibilities such as housing and benefit assessments, identifying housing needs, and liaising with local authorities to ensure service users are supported in their move on from the service.
On rota, you will work with individuals on their recovery, devising individually tailored support and risk management plans and ensuring effective discharge planning. You will help them gain skills and coping strategies while encouraging positive change and independence.
The role requires you to work with the team on a rota that may include evenings and weekends.
Ideally with experience in the mental health sector, it is essential that you have a good understanding of the recovery model, a solution-focused attitude, and a person-centred approach. You will be comfortable working independently and as part of a team, in a fast-paced environment and in challenging situations.
You should enjoy talking to people with different needs, be genuinely interested in helping them, and possess empathy, understanding, and patience.
Knowledge of housing pathways, benefits applications, and working with authorities such as social care is key for this role.
