We are looking to recruit a skilled and compassionate Social Worker to join our East Locality Assessment & Care Planning Team in Falkirk, supporting adults and their carers to live safely, independently, and with dignity.

The Role

  • Complete strengths-based assessments and develop outcome-focused care and support plans
  • Support adults and their carers to maintain independence and wellbeing
  • Work closely with health colleagues and partner agencies in a multi-disciplinary setting
  • Manage a varied caseload, including complex needs
  • Contribute to adult protection work where required
  • Maintain accurate and timely case recording and reports

What We’re Looking For

  • Degree or equivalent qualification in Social Work
  • SSSC registration (or eligibility)
  • Experience in adult social work (community-based experience desirable)
  • Strong communication, assessment, and risk management skills
  • A person-centred, strengths-based approach to practice
