We are looking to recruit a skilled and compassionate Social Worker to join our East Locality Assessment & Care Planning Team in Falkirk, supporting adults and their carers to live safely, independently, and with dignity.
The Role
- Complete strengths-based assessments and develop outcome-focused care and support plans
- Support adults and their carers to maintain independence and wellbeing
- Work closely with health colleagues and partner agencies in a multi-disciplinary setting
- Manage a varied caseload, including complex needs
- Contribute to adult protection work where required
- Maintain accurate and timely case recording and reports
What We’re Looking For
- Degree or equivalent qualification in Social Work
- SSSC registration (or eligibility)
- Experience in adult social work (community-based experience desirable)
- Strong communication, assessment, and risk management skills
- A person-centred, strengths-based approach to practice
