Hospital Team Social Worker Location: Stirling, Scotland Rate: £36–£40 per hour Job Type: Temporary / Contract Hours: Full-time Are you an experienced and compassionate Social Worker looking for your next opportunity within a hospital setting? We are currently seeking a skilled Hospital Team Social Worker to join a busy multidisciplinary team in Stirling, supporting patients and their families to achieve safe, appropriate and person-centred outcomes. The Role As a Hospital Team Social Worker, you will work closely with patients, families, carers and a range of health and social care professionals to assess needs, coordinate support and facilitate safe and timely discharge from hospital. Your responsibilities will include: * Undertaking comprehensive assessments of patients' social care needs. * Developing person-centred care and support plans. * Supporting patients through the hospital discharge process. * Assessing risks and identifying appropriate care and support arrangements. * Working closely with medical, nursing, occupational therapy and other multidisciplinary colleagues. * Liaising with families, carers and external agencies to coordinate services. * Arranging appropriate community-based support, care packages and placements where required. * Contributing to safeguarding assessments and risk management. * Advocating for patients' rights, choices and independence. * Maintaining accurate and up-to-date case records. * Participating in multidisciplinary meetings and discharge planning discussions. * Ensuring practice is in line with relevant legislation, policies and professional standards. About You We are looking for a qualified Social Worker with: * A recognised Social Work qualification. * Current registration with the Scottish Social Services Council (SSSC). * Experience within hospital, adult services, community social work or a similar setting. * Strong assessment, care planning and risk management skills. * Experience working within a multidisciplinary environment. * Knowledge of adult social care legislation, safeguarding and hospital discharge processes. * Excellent communication and interpersonal skills. * The ability to manage a busy caseload and prioritise effectively. * A professional, compassionate and person-centred approach. * The ability to work confidently both independently and as part of a wider team. Previous experience of hospital discharge or integrated health and social care services would be highly desirable.
