We are recruiting a Social Worker to join the Reablement & Intermediate Care Team (RIACT) in Darlington to support individuals to return home safely and regain independence.

The Role
You’ll work within the RIACT team, supporting safe and timely discharges from hospital and ensuring individuals receive the right level of care post‑discharge. This is a fast‑paced, rewarding role where your assessments directly impact recovery and long‑term outcomes.
Key Responsibilities
- Complete strengths‑based assessments to support discharge planning
- Facilitate safe and timely hospital discharges
- Arrange intermediate care and longer‑term support services
- Carry out post‑discharge reviews to assess ongoing needs
