About the Role

We\’re seeking dedicated and proactive social work professionals to join our two D2A teams supporting the areas of Rayleigh/Rochford and Castle Point/Benfleet.

These roles focus on timely hospital discharges and short-term interventions without a safeguarding remit, allowing for a focused and efficient approach to care planning and assessments. You’ll also work closely with Southend partners for joined-up care delivery.
