- Pay Rate: £38per hour
- Working Pattern:Hybrid – typically 2 days office-based and 3 days working from home / undertaking visits
- Contract:Full Time
About the Role:
We are seeking an experienced and committed Social Worker – Adult Servicesto join a multi-disciplinary team supporting adults with a range of social care needs, including individuals withLearning Disabilities.
This is an excellent opportunity for a qualified Social Worker with strong assessment, partnership-working and supervisory experience who is passionate about promoting independence, wellbeing and positive outcomes for adults.
Key Responsibilities:
- Undertake assessments, reviews and social care planning in accordance with relevant legislation, policies and professional standards.
- Work effectively within amulti-disciplinary teamto assess needs, manage risk and develop appropriate support plans.
- Work collaboratively with Social Care Services, Health Services, voluntary organisations, Education and other partner agencies.
- Support adults to maximise their independence, choice, wellbeing and quality of life.
- Maintain accurate, timely and comprehensive case records and effectively use relevant IT systems.
- Apply professional knowledge and judgement when managing complex cases and competing priorities.
- Provide appropriate supervision, guidance and support to colleagues where required.
- Keep up to date with changes in legislation, government guidance and developments across a broad range of social care issues.
- Identify and utilise appropriate social care resources and services beyond statutory provision.
