Senior Social Worker needed for our Hospital Team in East Riding, paying £37ph!
About the Role
East Riding is recruiting Senior Adults Social Workers to join its Hospital Social Work Team. These roles are critical in supporting timely and safe discharges while ensuring statutory duties are met for residents of the East Riding.
Key Responsibilities
- Completing Care Act Assessments in line with statutory responsibilities
- Undertaking Mental Capacity Act (MCA) assessments
- Working closely within multi-disciplinary teams (MDTs) across acute hospital settings
- Managing a varied and demanding caseload within a fast-paced hospital environment
- Supporting safe discharge planning and positive outcomes for adults
