Tripod Partners is seeking a SEND Key Worker to undertake statutory assessments and develop and maintain Education, Health and Care Plans (EHCP) across Cheshire East Council. You will manage a caseload of Children with SEND, attend annual reviews and modify plans as required, while liaising with childcare, education and vocational providers to source suitable placements for young people with EHCPs.
You will co-ordinate a person centred, outcome focused multi-agency approach, act as the Named contact.
