The role involves MDT collaboration, discharge facilitation, and ensuring timely hospital discharges with appropriate care arrangements.
You will work across a 7-day shift pattern, liaising with health professionals, patients, families and external agencies, and applying the Care Act and Mental Capacity Act to CHC processes.
