Job Description:
An exciting opportunity has arisen for an experienced Adults Social Worker to join the Integrated Discharge Team based at Scunthorpe General Hospital.
Working as part of a Multi-Disciplinary Team (MDT), you will play a key role in supporting effective and timely hospital discharges. You will be responsible for completing Discharge Facilitation Forms and ensuring that appropriate care and support arrangements are in place for adults leaving hospital.
This role involves working a shift pattern across 7 days, providing flexibility and continuity of support to patients and the wider MDT.
Key Responsibilities:
- Complete Discharge Facilitation Forms as part of the hospital discharge process.
- Work collaboratively as a member of the MDT to support safe, effective and timely discharges.
- Assess adults' care and support needs in line with relevant legislation and best practice.
- Contribute to discharge planning and identify appropriate care and support solutions.
- Liaise with health professionals, patients, families, carers and external agencies.
- Understand and apply the principles of the Care Act and Mental Capacity Act.
- Complete and understand NHS Continuing Healthcare (CHC) checklists and contribute to the CHC process where appropriate.
- Understand how to commission appropriate care and support services.
- Promote person-centred practice, independence, choice and wellbeing.
- Maintain accurate and timely case records and documentation.
