About the role:
This is a fantastic opportunity for an experienced Social Worker to join our Young Adults Transitions Team, supporting young people as they move from Children's Services into Adult Social Care.
Our service provides a truly multidisciplinary approach, bringing together Social Workers, Occupational Therapists, Social Care Practitioners and Occupational Therapy Aides to provide coordinated, person-centred support.
As a Social Worker within the service, you will play a key role in ensuring the best possible outcomes for young people aged 14-25, supporting and empowering young people and their carers to develop the skills, confidence and resources needed to achieve greater independence.
Key Responsibilities;
- Work with young people aged 14-25 who are transitioning between Children's Services and Adult Social Care.
- Lead on Care Act assessments, support conversations and support planning.
- Work alongside young people, families and carers to identify strengths, aspirations, needs and desired outcomes.
- Support young people to develop independent living skills at the right time and in a way that reflects their individual circumstances.
- Develop effective, person-centred support plans that promote independence and wellbeing.
- Identify opportunities for non-commissioned support, assistive technology, equipment and wider community resources to help young people achieve their goals.
- Where appropriate, support the commissioning of services to meet identified eligible needs.
- Contribute to the service's Early Help model within the Transitions Hub.
- Work collaboratively with colleagues across Social Care, Health and other partner services.
- Promote a consistent and strengths-based approach to assessments, planning and transition.
- Ensure young people and their carers are actively involved in decision-making and planning for their future.
- Maintain accurate and timely case records in line with statutory and organisational requirements.
