About the Role
We are seeking a skilled and committed Social Worker to join the All Age Disability (AAD) Service, working within the Young Adults Team (ages 16–25). This is a specialist role for a practitioner with experience across Children’s and Adults’ Services, with a strong understanding of relevant legislation and essential experience in transitions. The AAD Service aims to make a positive difference to children and young people aged 0–25 with Special Educational Needs (SEN) and additional needs, supporting them to achieve the best possible outcomes as they move into adulthood.
Main Responsibilities
- Support young people through the transition from Children’s to Adult Services
- Undertake statutory social work duties under both Children’s and Adult legislation
- Work collaboratively with Children’s Services, Adult Services, SEND, health partners, schools, and colleges
- Ensure young people and their families are fully involved in planning and decision-making
- Use Liquidlogic across both children’s and adults’ systems
- Contribute to multi-agency planning to ensure young people have access to a full range of opportunities
About the Service
The Young Adults Team supports young people with disabilities aged 16–25 as they prepare for adulthood across key life areas, including:
- Education, training, and employment
- Independent living
- Community inclusion
- Health and wellbeing
The work is primarily with young people with Learning Disabilities, alongside some young people with physical disabilities or complex health needs, supporting their transition from Children’s Services into adult life.
