About The Role
Denbighshire is recruiting an experienced Social Worker to join the Locality North Team. This is an excellent opportunity to deliver high-quality social work services to adults and their families, supporting positive outcomes in line with the service’s vision and priorities. The role focuses on person-centered practice, empowering individuals while ensuring safety, wellbeing, and independence are maintained.
Key Responsibilities
- Provide a high-quality social work service to adults and their families
- Undertake assessments, care planning, and reviews in line with statutory responsibilities
- Support individuals to build resilience and maintain independence
- Ensure adults and families are central to decision-making about their lives and desired outcomes
- Carry out safeguarding duties, balancing empowerment with risk management
- Develop an understanding of the local community’s culture, health, and social care needs to inform practice
- Engage in preventative work to reduce the need for long-term care or accommodation
- Arrange and support placements where required, including future planning for individuals
- Work collaboratively with multi-agency partners to deliver effective support
Requirements
- Qualified Social Worker with relevant degree or equivalent
- Registered with Social Care Wales (or eligible to register)
- Experience working with adults in a statutory setting
- Strong assessment, care planning, and safeguarding skills
- Ability to work autonomously and manage a varied caseload
- Excellent communication and interpersonal skills
