Qualified Social Worker – Adults East Riding of Yorkshire Council – Kingfisher Lodge, HU18 1LP Temporary Contract
Tripod Partners are currently recruiting for an experienced Qualified Adults Social Worker to join East Riding of Yorkshire Council.
This is an excellent opportunity for a Social Worker with strong Adult Social Care experience who is confident managing complex and varied caseloads.
Your responsibilities will include:
- Completing Care Act 2014 assessments and reassessments
- Undertaking strengths-based assessments
- Managing complex and varied caseloads
- Developing and reviewing person-centred support plans
- Promoting independence, wellbeing, choice and control
- Completing risk assessments and risk management plans
- Undertaking Mental Capacity Act assessments
- Facilitating Best Interests meetings and decisions
- Undertaking Section 42 safeguarding enquiries
- Managing concerns relating to abuse, neglect, self-neglect and exploitation
- Completing carers' assessments
- Arranging and reviewing care and support packages
- Working with residential, nursing, domiciliary and community care providers
- Working collaboratively with health, housing and voluntary sector partners
- Supporting hospital discharge and safe transitions back into the community
- Supporting adults with physical disabilities, learning disabilities, mental health needs and long-term conditions
- Maintaining accurate case records and professional reports
- Responding to urgent referrals and crisis situations
- Advocating for service users and ensuring their voices are heard
About You
- A recognised Social Work qualification
- Current Social Work England registration
- Strong Adult Social Care experience
- Experience completing Care Act assessments
- Knowledge of the Mental Capacity Act
- Experience undertaking safeguarding enquiries
- Strong risk assessment and management skills
- Experience managing complex cases
- Excellent communication and organisational skills
- The ability to work independently and adapt to changing demands
