Role Overview

We are seeking an experienced and motivated Qualified Social Worker to join a dynamic Adult Social Care team in Bournemouth. This is an excellent opportunity for a skilled practitioner who is passionate about delivering person-centred, strengths-based support to adults and young people transitioning into adult services.

Key Responsibilities

  • Complete robust Mental Capacity Assessments in line with the Mental Capacity Act (2005) and apply Best Interest decision-making frameworks where appropriate.
  • Undertake Care Act (2014) compliant assessments, support planning, reviews, and risk management.
  • Manage complex caseloads while exercising sound professional judgement within legislative and policy frameworks.
  • Produce high-quality, analytical written reports, assessments, and case records.
  • Prepare court reports and statements where required, including documentation for legal proceedings and the Court of Protection.
  • Support young people with EHCPs through transition into Adult Social Care, working collaboratively with education, health, and partner agencies.
  • Maintain accurate and timely records using Mosaic or equivalent case management systems.
  • Build positive and effective relationships with individuals, families, carers, and multi-agency professionals.
