About the Role
Somerset is currently seeking an experienced and motivated Adult Social Worker to join their dynamic Adult Social Care Team. This role involves working with adults who require care and support, helping them to live as independently as possible while safeguarding their wellbeing.
Key Responsibilities
- Undertake strengths-based assessments and create person-centred support plans
- Manage a caseload involving complex needs, including mental health, physical disabilities, older people, and learning disabilities
- Apply the Care Act 2014, Mental Capacity Act, and other relevant legislation
- Work collaboratively with health partners, care providers, families, and other agencies
- Support service users to access services, direct payments, and other resources
