About the role:
You will work with adults in the community, supporting individuals to maximise their independence, wellbeing and quality of life. The role will involve carrying out assessments, developing care and support plans, coordinating services and working closely with individuals, families and partner agencies.
Key Responsibilities;
- Complete Care Act assessments and reviews.
- Develop person-centred care and support plans.
- Safeguard adults at risk and respond appropriately to concerns.
- Work collaboratively with health and social care professionals.
- Support adults to maintain independence and achieve positive outcomes.
- Manage a varied caseload effectively.
- Maintain accurate and timely case records.
- Participate in relevant meetings and multi-disciplinary working.
