About the Role:
We are currently seeking an experiencedAdult Social Workerto joinEast Riding of Yorkshire Councilon an agency basis. This is an excellent opportunity for a skilled social worker to manage a complex and varied caseload while delivering high-quality, strengths-based, person-centred support to adults with care and support needs.
You will play a key role in promoting independence, wellbeing, choice and control, helping individuals achieve positive outcomes and remain safely within their communities.
Key Responsibilities:
- UndertakeCare Act 2014 assessments and reassessments, identifying eligible needs and desired outcomes.
- Complete strengths-based assessments and develop person-centred support plans.
- Manage a complex and varied caseload, exercising professional judgement in high-risk and challenging situations.
- Promote independence, wellbeing, choice and control for adults and their families.
- Conduct risk assessments and develop appropriate risk management plans.
- UndertakeMental Capacity Act (MCA) assessmentsand participate in Best Interests meetings and decision-making.
- Complete safeguarding enquiries underSection 42 of the Care Act, investigating concerns relating to abuse, neglect, self-neglect and exploitation.
- Develop, implement and monitor safeguarding plans to reduce risk and promote safety.
- Undertake carers' assessments and identify appropriate support for unpaid carers.
- Arrange, coordinate and review care and support packages, working closely with care providers.
- Liaise with residential, nursing, domiciliary and community care services.
- Work collaboratively with health professionals, housing services, voluntary organisations and other partner agencies.
- Attend multidisciplinary meetings, case conferences, safeguarding strategy meetings and reviews.
- Support hospital discharge planning and facilitate safe transitions back into the community.
- Help prevent unnecessary hospital admissions through early intervention and community-based support.
- Advocate for service users and ensure their views, wishes and desired outcomes remain central to decision-making.
- Support adults experiencing self-neglect, hoarding behaviours and other complex needs.
- Work with adults with physical disabilities, learning disabilities, mental health needs and long-term conditions.
