Our client in Essex are looking for 2 physiotherapists for the below role:
- Physiotherapist x2
- Band 6 positions
- Mon-Fri
You will be responsible for undertaking assessment and intervention in the community and care homes for patients under the Discharge to Assess (D2A) pathways. This includes:
- Assessing patient’s therapy and care needs
- Providing intervention to promote safety and independence
- Aiming to reduce the risk of re-admission or harm
- Helping support people onto the best pathway for their needs
- Developing, implementing, and evaluating care and therapy plans
- Assessing equipment needs
- Supporting discharge planning
- Working on rehabilitation goals to promote function and independence
The post holder will work autonomously within professional guidelines and the overall framework of the service’s policies and procedures. Duties will include assessing for health or social care packages to meet the patient’s needs. All these skills are competency led, and appropriate training will be provided where needed.
Due to the nature of the role, candidates will be required to hold a valid UK driving licence and access to their own car with appropriate insurance, as frequent travel for visits will be required for this role.
