Experienced Adults' Social Worker needed in a Hospital Team in Peterborough. Paying £34 ph!
Location: Peterborough City Hospital
Pay Rate: £34 per hour
Hours: 37 hours per week
Working Pattern: Monday–Friday, 5 days per week
About the Role:
We are currently seeking an experienced Social Worker to join the team based at Peterborough City Hospital.
This is an excellent opportunity for a dedicated Social Worker to work within a multidisciplinary environment, assessing the needs of individuals and ensuring they receive appropriate, person-centred support and services.
You will manage a varied caseload, including individuals with complex needs and safeguarding concerns, while working collaboratively with service users, carers and other professionals.
Requirement:
- Assess the needs of individuals who potentially or currently meet the eligibility criteria for services.
- Formulate and develop appropriate service plans and care plans.
- Oversee the implementation of care and support plans, ensuring services are delivered effectively.
- Monitor and regularly review individuals' ongoing needs and the appropriateness of services provided.
- Responsibly manage a varied and complex caseload, including Adult Safeguarding cases.
- Ensure assessments, reviews and interventions are completed within required timescales.
- Work in accordance with relevant legislation, policies, procedures and professional standards.
- Work collaboratively with service users, carers and colleagues across multidisciplinary teams.
- Assess needs, plan and deliver appropriate services across areas including:
- Sensory Impairment
- Physical Disability
- Learning Disability
- Maintain accurate and timely case records and documentation.
- Promote person-centred practice, independence, wellbeing and positive outcomes for service users.
- Liaise effectively with health and social care professionals and other partner agencies.
