Responsibilities
- To assess the needs of people and carers with regards to the provision of moving and handling or other specialised transfer equipment to support the safe delivery of care packages to ensure that people can remain safe in their own home.
- Following assessment, write comprehensive moving and handling plans / risk assessments to inform Care and Support teams, community health professionals and domiciliary care providers (internal and external) of the requirements for the safe delivery of a care package. This will also inform the resultant Care and Support plan.
- To undertake person centred functional assessments and provide recommendations for the provision of equipment and low/medium level adaptations
- Undertake an assessment of a person\\\'s circumstances in relation to their difficulties accessing suitable parking to inform the annual Disabled Parking Bay Scheme allocation panel.
