Tripod are looking for a dedicated and experienced Occupational Therapist to join their community team supporting adults with illness, disability, and rehabilitation needs across the county. This is an excellent opportunity for an Occupational Therapist who is passionate about enabling people to maintain independence and remain safely within their own homes through specialist assessments, equipment provision, and home adaptations.

Key Responsibilities

  • Assess the needs of adults with disabilities or long-term conditions within the community
  • Recommend and arrange specialist equipment and home adaptations
  • Support hospital discharge planning and rehabilitation pathways
  • Provide advice and guidance to clients, families, and carers
  • Liaise with Housing, NHS teams, voluntary organisations, architects, and external agencies
  • Assess moving and handling requirements and provide appropriate recommendations
  • Ensure completed adaptations meet assessed needs
  • Work collaboratively to secure funding where required through charities, NHS, or partner agencies
  • Maintain up-to-date knowledge of Occupational Therapy equipment and best practice

Requirements

  • Qualified Occupational Therapist with relevant professional registration
  • Experience working within community or adult social care settings
  • Strong assessment and case management skills
  • Excellent communication and partnership-working abilities
  • Knowledge of equipment provision, moving and handling, and adaptation processes
  • Ability to manage a varied caseload independently
